Welcome to “Wellness Works 2009” the City of Show Low’s employee wellness program.  The purpose of this program is to encourage employees to get out, get active and be healthy in all facets of life. 

     Each category has a point value and a maximum number of points allowed for each category.  As with the previous program a wellness prize will be awarded monthly for achieving the minimum number of 100 points.  This year we will also give a bonus prize to the top point earners for the month. 

     Participating is easy.  Just keep track of each activity.  At the end of the month interoffice your form or email your points to Mary Atteberry at City Hall. 

     At the end of 12 months we will award prizes in three different categories:  

                                    1200 points – Super cool Wellness T-Shirt (100 points per month average)

                                    1800 points – Wellness sweatshirt or fleece (150 points per month average)

                                    2400 points - $50 gift certificate (200 points per month average)

     So remember:  100 wellness points each month, turn your form in to Mary Atteberry by the 10th of the month.  Piece of cake fruit!!  If you have questions, or need a Wellness Works packet call Mary @ ext. 4080.
